John Burns suffocated after being crushed under a large quantity of rice, which caused severe injuries to his chest. According to the British newspaper "Daily Star," the company responsible for the incident was fined £1.8 million for its liability in crushing the man under 30 tons of rice.
The company, "Associated British Ports Holding," admitted its responsibility for the incident, acknowledging several safety-related issues concerning its workers. A statement from the British Health and Safety Agency noted that Burns, along with three other drivers, was transporting rice from a container ship to a storage warehouse. When Burns's vehicle did not exit the warehouse, officials inspected the site and found him buried under a huge pile of rice. According to the coroner’s report, Burns suffered severe injuries to his legs and chest, and the cause of death was determined to be asphyxiation.
